This guide consists entirely of candidates’ responses to the questionnaires as submitted,
except for the use of standard abbreviations and for editing when the total exceeded the word limit.
Questionnaires were sent to the candidates via first-class mail and via this website.
If a questionnaire was not received back from a candidate, his or her name appears with the words
“No information available.”
